South Florida Area 15 Group Information Change Form

Please use this form if your registered group wants to make a change in the information listed in the South Florida Area 15 registrar database as well as in Fellowship Connection, the General Service Office’s database, which will ensure that you continue to stay connected to your district, to South Florida Area 15, and to the General Service Office. Examples of changes include change in meeting place; change in General Service Representative, Alternate General Service Representative, or group contact; or change in group type (in person, online, hybrid).

PLEASE NOTE: Completing and submitting this form will NOT change how your group appears in your local meeting list or in the online Meeting App.
